If this is your FIRST SESSION with me, there are some additional things that are necessary. Please follow the steps 1, 2 and 3 below well in advance of your first Session. Step 4 is about connecting with me for your session.
If this is a FOLLOW-UP SESSION*, please follow step 1 only. It isn’t necessary for you to fill out the forms again. Thank you!
Below are the Steps YOU need to do BEFORE Our Session.
Make a Payment for your Session
Payment must be received PRIOR to your session. Please make your Payment via PayPal by clicking on a button below. PayPal is free and very secure. NOTE: YOU WILL BE DIRECTED to PAYPAL and then RETURNED to this page to continue with Step #2.
Individual Session – $200 per hour
Fill Out the Client Agreement, Disclosure, Waiver and Release Form
This form is a required by Dr. Raj as a Certified Hypnotist & Past Life Regression Healer. It is a signed agreement. You can either fill it out ONLINE or PRINT OUT and then scan & email or bring it with you.
Fill Out Your Client Information Profile
This form is also required by Dr. Raj as a Certified Hypnotist & Past Life Regression Healer. It is a questionnaire that helps him and you to focus in on what is best for your Session(s). You can either fill it out ONLINE or PRINT OUT and then scan & email or bring it with you.
Connect for Our Session
We use two methods for individual sessions and you can choose which is best for you and your circumstances. Decide before hand which on you will use and feel comfortable using. MAKE SURE YOU HAVE PAID FOR YOUR SESSION and FILLED OUT YOUR FORMS.
- Phone Session:Use the call-in number below to connect with me for a phone session. Please, place your call at the time given for your appointment.Call-in number: +1 408-341-9170
- Video Conferencing: Using Zoom (very simple 1-time setup). Click on the link below and follow the directions for use. On your first session, you will be asked you to download the ZOOM software to connect for video conferencing. You will only need to do this once. The “Reminder” email/text sent 24 hrs prior to your appointment will again give you the instructions for BOTH methods. Should you need to cancel, please allow 24 hrs if not more and use the link in your confirmation email or contact me by email using my contact form. I appreciate you letting me know in advance and maybe we can then reschedule.
